I don't think anybody knows for sure. That piece of information has been
in the FAQ since the question on Smithing was added in February 21th
1997. Interestingly, that's one day before rec.games.roguelike.adom came
into existence. Originally, the FAQ entry was phrased like this:

# *2.15: How do I use my Smithing skill?
#
# Smithing is a complicated task. You need the following ingredients:
#
# 1) A one-handed hammer. Warhammers work nicely.
# 2) An anvil. I don't think there are any guaranteed anvils in the
# game, but there are a few places where they have a high probability
# of being dropped by dead monsters.
# 3) Something to smith! You need to know what kind of metal it's made
# of.
# 4) An ingot of the appropriate metal. A lump of ore won't cut it.
# 5) A forge. You can't carry these around with you, but you can hope
# to find one in your travels. There are a couple of guaranteed
# forges.
#
# Stand on the forge, wield the hammer, and apply your Smithing skill to
# your metal item. (This will also serve to make metal ingots out of
# ore lumps.)
# Your Metallurgy skill seems to affect your chance of success; I've
# also heard whispers that wearing a leather apron helps in some
# mysterious way. (That's old information, however, based on a
# half-remembered post from Thomas which I skimmed over about 6 months
# ago...)

I could not find any traces of that "half-remembered post" referred too,
however. Maybe it's just a myth?

No, I recall the post too. Someone conducted a load of research
into things like the effect of an apron or not, even the b/u/c
status of things like the apron, anvil and ingots. IIRC, an apron
helped especially when converting ore to ingots whilst still at
at low smithing skill. Shame I cannot recall the title :(

I recall that post too but it was fairly recent, so it can't be the post
referred to. The title of this recent one is "Smithing trials (long,
spoily and now wrapped properly)".

I think there is another, older one. Unless my memory is bad :)
